Mental Health and Drug & Alcohol Evaluations

 

If a court, attorney, probation officer, or employer has required you to complete a Mental Health Evaluation or Drug and Alcohol Evaluation, Grace Therapy provides professional assessments for adults throughout Chicago and across Illinois.

 

Evaluations are completed during a one hour secure online appointment with a licensed clinician. During the session, a comprehensive clinical interview is conducted and symptoms are reviewed using DSM-5-TR diagnostic criteria when appropriate. Following the appointment, the completed evaluation and clinical recommendations can be provided for legal documentation.

 

Insurance may be accepted when applicable, and self pay options are available. For clients choosing self pay, the fee is $200 for a Drug and Alcohol Evaluation and $400 for a Mental Health Evaluation.
