Gender-Affirming Surgery Evaluations in Illinois

Grace Therapy provides psychological evaluations for adults (18+) seeking gender-affirming surgery in Illinois. Our licensed clinicians offer respectful, affirming, and clinically sound evaluations for transgender, non-binary, genderqueer, and gender-expansive individuals.

In addition to evaluation services, Grace Therapy also provides individual, couples, and group therapy for clients seeking ongoing mental health support.

What Is a Gender-Affirming Surgery Evaluation?

Many surgical providers and insurance plans require a letter from a licensed mental health professional before approving gender-affirming surgical procedures. These letters are typically based on guidelines established by the World Professional Association for Transgender Health (WPATH).

To obtain a letter, a client completes a psychological evaluation with a licensed mental health provider. This evaluation usually consists of a structured clinical interview and may take place over one or more sessions, depending on the individual situation and external requirements. Following the evaluation, the clinician may prepare a letter summarizing clinical findings and recommendations, when appropriate.

Are Evaluations Required for Hormone Replacement Therapy (HRT)?

In Illinois, many medical providers use an informed consent model for hormone replacement therapy. Under this model, a mental health evaluation or letter is often not required to begin HRT. Requirements vary by provider, so clients should confirm directly with their prescribing medical professional.

What Does the Evaluation Include?

Gender-affirming surgery evaluations focus on understanding the client’s experience and readiness for surgery while meeting applicable professional standards. Clinicians at Grace Therapy approach this process with care, neutrality, and respect for each individual’s lived experience.

Evaluation topics may include:

  • Gender identity development and personal history

  • Current and past mental health functioning

  • Social and emotional support systems

  • The specific surgical procedure being sought

  • Understanding of potential risks and benefits

There is no single narrative or timeline required to pursue gender-affirming care. Evaluations are conducted with the understanding that gender identity experiences vary widely and that emotional distress may be influenced by social, cultural, and environmental factors.
